Wellness Champions, LLC personal support services provide community integration and utilizes individually designed services to help people with disabilities live independently and engage in their community. The Participants receive varying degrees of support, that enhance the participant’s opportunity for community participation and to exercise choice and control over the participant’s own life.
With Personal Support Services, Wellness Champions employees provide a full range of community based support which includes enhanced development of the participant’s natural support system (family, friends and neighbors of the participant) for greater fulfillment of our cooperative mission: to ensure participants are fully assimilated and included in their neighborhoods, places of work, and places of worship.
